Aus der Wunde lief st\u00e4ndig ser\u00f6se Fl\u00fcssigkeit ab, und seit \u00fcber einem Monat gab es keine Anzeichen f\u00fcr eine Epithelisierung. Sie unterzog sich einer adjuvanten Strahlentherapie, die die Wundheilung weiter beeintr\u00e4chtigt. Die Standard-Wundversorgung mit modernen Verb\u00e4nden und topischen Mitteln f\u00fchrte nicht zu einem Heilungsfortschritt. Bei ihr bestand ein hohes Risiko f\u00fcr eine Infektion und einen weiteren chirurgischen Eingriff.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Objektive Befunde:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Bewertung von Wunden:<\/strong>\n<ul class=\"wp-block-list\">\n<li><strong>Standort:<\/strong>\u00a0Linke Achselh\u00f6hle.<\/li>\n\n\n\n<li><strong>Gr\u00f6\u00dfe:<\/strong>\u00a04,0 cm x 2,5 cm.<\/li>\n\n\n\n<li><strong>Die Tiefe:<\/strong>\u00a00,4 cm.<\/li>\n\n\n\n<li><strong>Wundbett:<\/strong>\u00a080% blasser, nicht lebensf\u00e4higer gelber Schorf; 20% rotes Granulationsgewebe. Keine Anzeichen von Epithelmigration.<\/li>\n\n\n\n<li><strong>Exsudat:<\/strong>\u00a0M\u00e4\u00dfige Menge ser\u00f6ser Drainage.<\/li>\n\n\n\n<li><strong>Peri-Wund-Haut:<\/strong>\u00a0Erythemat\u00f6s (rot), mazeriert und zart.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Schmerz:<\/strong>\u00a0Bewertung 6\/10 an der Wundstelle, verschlimmert durch Armbewegung.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Behandlungsplan:<\/strong><br>Es wurde ein umfassendes Wundpflegeprotokoll eingeleitet, mit&nbsp;<strong><a href=\"https:\/\/fotonmedix.com\/de\/kategorie\/produkt\/\"   title=\"Produkte\"  data-wpil-monitor-id=\"2742\">Laser der Klasse 4<\/a> Therapie<\/strong>&nbsp;als prim\u00e4re biostimulierende Modalit\u00e4t.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Ger\u00e4t:<\/strong>\u00a0A\u00a0<strong><a href=\"https:\/\/fotonmedix.com\/de\/kategorie\/produkt\/\"   title=\"Produkte\"  data-wpil-monitor-id=\"2741\">Lasertherapie der Klasse 4<\/a><\/strong>\u00a0Ger\u00e4t mit zwei Wellenl\u00e4ngen (905nm gepulst, 810nm kontinuierlich).<\/li>\n\n\n\n<li><strong>H\u00e4ufigkeit:<\/strong>\u00a03 Mal pro Woche.<\/li>\n\n\n\n<li><strong>Protokoll:<\/strong>\n<ul class=\"wp-block-list\">\n<li>Die Wunde wurde zun\u00e4chst gereinigt und von losem Schorf befreit.<\/li>\n\n\n\n<li>Der Laser wurde auf zwei Arten eingesetzt: 1) Direkt auf das Wundbett in einem Gittermuster, wobei das Handst\u00fcck 1-2 cm \u00fcber das Gewebe gehalten wurde. 2) Um den Umfang der Wunde herum, um die Epithelmigration und den Lymphfluss zu stimulieren.<\/li>\n\n\n\n<li>Anf\u00e4nglich wurde eine niedrigere Dosis (4 J\/cm\u00b2) verwendet, um eine \u00dcberstimulation zu vermeiden, die dann schrittweise auf 6 J\/cm\u00b2 erh\u00f6ht wurde, sobald das Gewebe darauf ansprach.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Erg\u00e4nzende Betreuung:<\/strong>\u00a0Fortgesetzte Verwendung eines Hydrofaserverbands, um das Exsudat zu bew\u00e4ltigen und die Haut an der Wundumgebung zu sch\u00fctzen.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Ergebnisse und Resultate:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Nach 3 Behandlungen (1 Woche):<\/strong>\u00a0Das Wundbett zeigte eine signifikante Verbesserung mit einem R\u00fcckgang des Schorfs auf 30% und einer Zunahme des fleischigen roten Granulationsgewebes auf 70%. Die Drainage wurde reduziert.<\/li>\n\n\n\n<li><strong>Nach 6 Behandlungen (2 Wochen):<\/strong>\u00a0Die Wunde war 3,0 cm x 1,5 cm gro\u00df. Der Schorf war vollst\u00e4ndig entfernt. Es waren Epithelinseln sichtbar, und die Wundr\u00e4nder zogen sich deutlich zusammen (Wundkontraktion).<\/li>\n\n\n\n<li><strong>Nach 9 Behandlungen (3 Wochen):<\/strong>\u00a0Die Wunde war 1,0 cm x 0,5 cm gro\u00df. Der Schmerz wurde mit 1\/10 bewertet.<\/li>\n\n\n\n<li><strong>Nach 12 Behandlungen (4 Wochen):<\/strong>\u00a0Die Wunde war vollst\u00e4ndig geschlossen und epithelisiert.<\/li>\n\n\n\n<li><strong>Nachbereitung:<\/strong>\u00a0Das geheilte Gewebe blieb w\u00e4hrend der restlichen Bestrahlungen stark und widerstandsf\u00e4hig.
