La articulaci\u00f3n est\u00e1 visiblemente inflamada y el tratamiento convencional ya lleva meses sin producir ning\u00fan cambio duradero.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El profesional sanitario aumenta la potencia del l\u00e1ser.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El paciente nota inmediatamente el calor.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Sin embargo, el objetivo m\u00e1s profundo no ha recibido necesariamente la misma proporci\u00f3n de energ\u00eda \u00fatil.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Esta es una de las contradicciones pr\u00e1cticas que subyacen a la terapia con l\u00e1ser de alta intensidad. Sin embargo, requiere una dosificaci\u00f3n controlada, ya que la misma caracter\u00edstica de absorci\u00f3n que hace que la longitud de onda de 1470 nm sea biol\u00f3gicamente interesante tambi\u00e9n puede aumentar la acumulaci\u00f3n t\u00e9rmica local.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Es aqu\u00ed donde la potencia, el movimiento, la estructura del pulso, la zona de tratamiento y el ciclo de trabajo se vuelven indisolubles de la selecci\u00f3n de la longitud de onda.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Por qu\u00e9 la longitud de onda de 980 nm se comporta de forma diferente<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A aproximadamente 980 nm, la absorci\u00f3n del agua es considerablemente menor que a 1470 nm, mientras que la absorci\u00f3n de la hemoglobina sigue siendo significativa.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La relaci\u00f3n entre estos crom\u00f3foros se ha descrito ampliamente en la literatura m\u00e9dica sobre l\u00e1ser. Los estudios que comparan las longitudes de onda utilizadas para el tratamiento de tejidos distinguen entre las longitudes de onda m\u00e1s cortas del infrarrojo cercano \u2014como 810, 940, 980 y 1064 nm\u2014 y las longitudes de onda m\u00e1s largas, por encima de aproximadamente 1200 nm, en las que el agua se convierte en un absorbente mucho m\u00e1s dominante.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Esto hace que la longitud de onda de 980 nm tenga un perfil de tratamiento diferente.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">En el caso de un protocolo de rehabilitaci\u00f3n profunda, el profesional sanitario puede utilizar la longitud de onda de 980 nm cuando el objetivo sea lograr una penetraci\u00f3n \u00f3ptica m\u00e1s profunda combinada con una interacci\u00f3n vascular y fotot\u00e9rmica, mientras que recurrir\u00e1 a la longitud de onda de 1470 nm cuando resulte cl\u00ednicamente \u00fatil una mayor absorci\u00f3n de agua.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Por eso, un sistema de clase IV multilongitud de onda puede resultar m\u00e1s pr\u00e1ctico que una plataforma basada en una \u00fanica longitud de onda fija.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El tratamiento no tiene por qu\u00e9 obligar a todos los pacientes a seguir la misma estrategia \u00f3ptica.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">El caso que pone de manifiesto el problema<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Un caso cl\u00ednico de FotonMedix publicado por el fabricante describe a una paciente de 62 a\u00f1os con osteoartritis bilateral de rodilla de grado III seg\u00fan la escala de Kellgren-Lawrence.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El cuadro cl\u00ednico inicial inclu\u00eda una puntuaci\u00f3n de 8\/10 en la escala VAS de dolor, una flexi\u00f3n de la rodilla limitada a 95 grados y un derrame articular visible.
