La cuesti\u00f3n fundamental no es simplemente <em>si<\/em> el l\u00e1ser reduce la hernia, pero <em>por qu\u00e9<\/em> una reducci\u00f3n diminuta del volumen se traduce en un alivio significativo de los s\u00edntomas.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">El principio hidr\u00e1ulico del n\u00facleo pulposo<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">El disco intervertebral funciona como un sistema hidr\u00e1ulico cerrado. Leve hipoestesia sensitiva en el dedo gordo del pie izquierdo.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Diagn\u00f3stico preliminar:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La RMN confirm\u00f3 una hernia discal paracentral izquierda contenida en L4-L5, que comprim\u00eda la ra\u00edz nerviosa L5 transversal. La puntuaci\u00f3n de la EAV se redujo a 4\/10 (dolor en la zona de punci\u00f3n).<\/li>\n\n\n\n<li><strong>Semana 2:<\/strong> Dolor irradiado en la pierna completamente resuelto. SLR negativo hasta 80 grados.<\/li>\n\n\n\n<li><strong>Mes 3 (Conclusi\u00f3n):<\/strong> La IRM mostr\u00f3 una retracci\u00f3n del contorno herniado. La intensidad de la se\u00f1al del n\u00facleo se mantuvo sana. La paciente volvi\u00f3 a la actividad plena con una puntuaci\u00f3n de la EAV de 0\/10.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Nota cl\u00ednica:<\/strong> El \u00e9xito se bas\u00f3 en la naturaleza \u201ccontenida\u201d de la hernia. Si el anillo se hubiera roto (extrusi\u00f3n), la PLDD habr\u00eda sido ineficaz, ya que el mecanismo hidr\u00e1ulico falla en un sistema abierto.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">4. Una fibra de cuarzo est\u00e9ril y de alta eficacia de transmisi\u00f3n no es negociable.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Estabilidad de potencia y ancho de pulso<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">El dispositivo debe mantener una potencia en vatios estable. En PLDD, una fluctuaci\u00f3n de 2-3 vatios puede significar la diferencia entre vaporizaci\u00f3n y carbonizaci\u00f3n. El software debe permitir ajustes precisos de la duraci\u00f3n del pulso (por ejemplo, de 0,5 a 3 segundos). El \u201ctiempo de relajaci\u00f3n t\u00e9rmica\u201d (el intervalo entre pulsos) permite que el tejido se enfr\u00ede, evitando da\u00f1os t\u00e9rmicos acumulativos. Si no (secuestrado) -&gt; Microdiscectom\u00eda.<\/li>\n\n\n\n<li><strong>\u00bfSe conserva la altura del disco?<\/strong> Si el disco est\u00e1 colapsado (&lt;50%), no hay suficiente n\u00facleo para vaporizar.<\/li>\n\n\n\n<li><strong>\u00bfHay estenosis?<\/strong> Las estenosis \u00f3seas no pueden tratarse con l\u00e1ser de tejidos blandos.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Siguiendo estos estrictos criterios de inclusi\u00f3n, las tasas de \u00e9xito de la PLDD se aproximan al 80-85%, ofreciendo una r\u00e1pida reincorporaci\u00f3n al trabajo sin la formaci\u00f3n de tejido cicatricial asociada a la cirug\u00eda abierta.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">6. Conclusi\u00f3n<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La eficacia de <strong>Descompresi\u00f3n discal percut\u00e1nea con l\u00e1ser<\/strong> se basa en las leyes de la termodin\u00e1mica y la mec\u00e1nica de fluidos.
