Les premiers pas le matin sont douloureux, marcher apr\u00e8s \u00eatre rest\u00e9 assis devient p\u00e9nible, rester debout pendant plusieurs heures devient difficile, et l'augmentation de l'activit\u00e9 quotidienne peut faire r\u00e9appara\u00eetre imm\u00e9diatement la douleur au talon.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pour le clinicien, la difficult\u00e9 r\u00e9side dans le fait que le fascia plantaire n'est pas une cible superficielle \u00e0 laquelle on peut simplement appliquer davantage d'\u00e9nergie jusqu'\u00e0 ce que la douleur disparaisse.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La zone douloureuse situ\u00e9e autour du tubercule calcan\u00e9en m\u00e9dial comprend la peau, le tissu sous-cutan\u00e9, l'insertion du fascia plantaire, le tissu conjonctif et des structures m\u00e9caniques soumises \u00e0 de fortes contraintes. Le fascia plantaire est par ailleurs constamment soumis \u00e0 des contraintes de traction lorsque l'on se tient debout ou que l'on marche.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cela pose un probl\u00e8me tr\u00e8s sp\u00e9cifique pour la th\u00e9rapie au laser \u00e0 haute intensit\u00e9.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le traitement doit d\u00e9livrer une \u00e9nergie optique significative \u00e0 la zone cibl\u00e9e tout en \u00e9vitant toute accumulation thermique inutile en surface. Une puissance plus \u00e9lev\u00e9e peut r\u00e9duire la dur\u00e9e du traitement, mais cela ne signifie pas pour autant que les tissus plus profonds re\u00e7oivent une meilleure dose th\u00e9rapeutique.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">C'est \u00e0 ce moment-l\u00e0 que <strong>Qu'est-ce que la th\u00e9rapie au laser ?<\/strong> cette question s'av\u00e8re bien plus pertinente que de simplement se demander quelle est la puissance de la machine.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Un traitement \u00e0 haute intensit\u00e9 peut entra\u00eener des effets photobiologiques et thermiques, mais la r\u00e9ponse d\u00e9pend de la longueur d'onde, de l'absorption par les tissus, de la diffusion, de la zone trait\u00e9e, de la dur\u00e9e d'exposition, du mouvement de l'applicateur et de l'\u00e9nergie totale.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Une r\u00e9cente \u00e9tude clinique randomis\u00e9e, en double aveugle et contr\u00f4l\u00e9e par traitement simul\u00e9, publi\u00e9e en 2026, constitue un exemple particuli\u00e8rement instructif, car elle a test\u00e9 un protocole \u00e0 haute intensit\u00e9 clairement d\u00e9fini pour le traitement de la fasciite plantaire et a mis en \u00e9vidence un aspect que le marketing des \u00e9quipements m\u00e9dicaux \u00e9vite souvent de mentionner.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les deux groupes ont progress\u00e9.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le groupe ayant b\u00e9n\u00e9fici\u00e9 d'un traitement au laser \u00e0 haute intensit\u00e9 n'a pas pr\u00e9sent\u00e9 de b\u00e9n\u00e9fice suppl\u00e9mentaire statistiquement significatif par rapport au groupe ayant re\u00e7u un traitement au laser simul\u00e9, alors que les deux groupes avaient \u00e9galement effectu\u00e9 des \u00e9tirements standardis\u00e9s du fascia plantaire et du tendon d'Achille.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ce r\u00e9sultat ne rend pas pour autant cette technologie obsol\u00e8te.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cela nous apprend quelque chose de bien plus pr\u00e9cieux.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Un protocole de th\u00e9rapie au laser \u00e0 haute intensit\u00e9 doit \u00eatre \u00e9valu\u00e9 en fonction de la conception globale du traitement, et non pas uniquement en fonction de sa puissance de sortie.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Pourquoi la fasciite plantaire est-elle difficile \u00e0 traiter au laser ?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La fasciite plantaire est souvent d\u00e9crite comme une inflammation du fascia plantaire, mais la douleur chronique au talon d'origine plantaire est plus complexe que ne le laisse entendre cette simple appellation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Une fasciite plantaire chronique s'accompagne de modifications d\u00e9g\u00e9n\u00e9ratives, d'une alt\u00e9ration de l'organisation du collag\u00e8ne, de sollicitations m\u00e9caniques r\u00e9p\u00e9t\u00e9es et de modifications au niveau de l'insertion du fascia.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le patient peut pr\u00e9senter une douleur au niveau du fascia plantaire sans pour autant souffrir d'une affection purement inflammatoire.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cette distinction est importante au moment de choisir un traitement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Si le probl\u00e8me principal r\u00e9side dans une surcharge m\u00e9canique r\u00e9p\u00e9titive, le laser ne peut pas \u00e9liminer cette charge m\u00e9canique.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Si une raideur au niveau du mollet contribue \u00e0 une tension excessive du fascia plantaire, le traitement au laser ne suffit pas \u00e0 lui seul \u00e0 corriger la restriction sous-jacente.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Si le poids corporel, les chaussures, la dur\u00e9e de station debout ou le volume de course continuent \u00e0 surcharger les tissus, une diminution temporaire de la douleur ne r\u00e9sout pas la cause du probl\u00e8me.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">C'est pourquoi les \u00e9tirements et la gestion de la charge restent au c\u0153ur du traitement conservateur.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L'essai randomis\u00e9 de 2026 a d\u00e9lib\u00e9r\u00e9ment retenu les \u00e9tirements du fascia plantaire et du tendon d'Achille comme traitement de base commun aux deux groupes. Les chercheurs souhaitaient d\u00e9terminer si l'ajout de la HILT produisait un effet suppl\u00e9mentaire par rapport \u00e0 cette intervention conservatrice.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cette conception pr\u00e9sente un int\u00e9r\u00eat clinique, car elle refl\u00e8te ce qui se passe dans un v\u00e9ritable service de r\u00e9\u00e9ducation.<\/p>\n\n\n\n<p de la th\u00e9rapie au laser sur le fascia plantaire ?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La r\u00e9ponse biologique \u00e0 la lumi\u00e8re th\u00e9rapeutique d\u00e9pend de la quantit\u00e9 d'\u00e9nergie optique qui atteint les tissus et de la mani\u00e8re dont cette \u00e9nergie interagit avec les chromophores cellulaires.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u00c0 des intensit\u00e9s plus faibles, la recherche sur la photobiomodulation s'est largement concentr\u00e9e sur la signalisation cellulaire, l'activit\u00e9 mitochondriale, les voies de l'oxyde nitrique, la signalisation inflammatoire et la r\u00e9paration tissulaire.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u00c0 des intensit\u00e9s \u00e9lev\u00e9es, les effets thermiques prennent de plus en plus d'importance.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cela ne signifie pas pour autant que tout traitement \u00e0 haute intensit\u00e9 se r\u00e9sume \u00e0 une simple proc\u00e9dure de chauffage.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cela signifie que le clinicien doit g\u00e9rer deux effets th\u00e9rapeutiques qui interagissent.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L'\u00e9nergie optique peut stimuler des processus biologiques.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L'\u00e9nergie absorb\u00e9e peut \u00e9galement faire capable de la produire.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Un traitement contr\u00f4l\u00e9 de 7 W ou 12 W peut s'av\u00e9rer plus appropri\u00e9 qu'une puissance plus \u00e9lev\u00e9e si l'on tient compte de la zone de traitement, de la dur\u00e9e d'exposition et de la r\u00e9ponse tissulaire.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">C'est l'une des raisons pour lesquelles un professionnel <strong>appareil de th\u00e9rapie au laser<\/strong> n\u00e9cessite une puissance de sortie r\u00e9glable.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Pourquoi l'\u00e9nergie optique diminue-t-elle avec la profondeur des tissus ?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Lorsque l'\u00e9nergie laser p\u00e9n\u00e8tre dans un tissu biologique, elle ne conserve pas ind\u00e9finiment son intensit\u00e9 initiale.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L'absorption \u00e9limine une partie de l'\u00e9nergie optique.<\/p>\n\n\n\n<p fascia also have their own optical characteristics.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The result is not a sharp boundary where the laser suddenly stops.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is a gradual decline in usable optical energy.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is why \u201c15 cm penetration\u201d should not be interpreted as meaning that the same therapeutic energy reaches tissue 15 cm below the skin.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">FotonMedix describes LaserMedix-MAX with a peak penetration-depth maintaining technology and states a tissue penetration depth of up to 15 cm in its product information. The same platform provides five wavelengths at 650 nm, 810 nm, 915 nm, 940 nm and 980 nm, with a stated maximum output of 30 W.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For a clinician, the useful interpretation is not a single penetration number.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The useful question is whether sufficient energy can reach the intended tissue while keeping superficial thermal exposure under control.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why 1064 nm Is Used in Published HILT Protocols<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The strongest plantar fasciitis evidence available for a clearly defined high-intensity protocol uses 1064 nm.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The 2026 randomized trial used 1064 nm in continuous mode at 12 W for 250 seconds, with an energy density of 120 J\/cm\u00b2 over a 25 cm\u00b2 treatment area.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That produced 3,000 J per session.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nine treatment sessions were performed over three weeks, with one session every three days.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The treatment was applied with a 10 mm pen applicator over the calcaneal insertion and along the medial border of the plantar fascia while the foot was positioned neutrally.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This protocol is useful because every important parameter is visible.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The clinician knows:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>longueur d'onde<\/li>\n\n\n\n<li>output power<\/li>\n\n\n\n<li>treatment mode<\/li>\n\n\n\n<li>exposure time<\/li>\n\n\n\n<li>energy density<\/li>\n\n\n\n<li>treatment area<\/li>\n\n\n\n<li>total energy<\/li>\n\n\n\n<li>number of sessions<\/li>\n\n\n\n<li>treatment interval<\/li>\n\n\n\n<li>application region<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">That is far more informative than saying \u201chigh-power laser was used.\u201d<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The 2026 Clinical Case Evidence<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The latest randomized clinical evidence included 36 enrolled participants, with 34 completing the study.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The HILT group contained 18 patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There were 16 women and 2 men.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The mean age was 46 years with a standard deviation of 11.4 years.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The mean duration of foot pain was 14.7 weeks.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The study involved unilateral plantar fasciitis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The trial excluded patients with conditions that could substantially change the interpretation of plantar heel pain, including previous foot surgery, inflammatory arthritis, sensory disorders associated with diabetes, recent corticosteroid injection into the plantar fascia, neuropathic foot pain, intrinsic foot muscle weakness and pregnancy.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That selection is important.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A clinical laser study is only as useful as the patient population it actually treats.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If a patient has neuropathic pain rather than plantar fascia pathology, treating the heel with more optical energy does not address the real problem.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Published Clinical Treatment Record<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The following table uses the actual published HILT cohort and protocol. That cumulative value is a calculation rather than a separate published outcome.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The important clinical result is that the HILT group improved significantly from baseline, but the sham group also improved, and the between-group differences were not statistically significant.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Pain Data Are More Interesting Than the Laser Setting<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The HILT group had a mean post-treatment VAS score of 23.0 mm compared with 26.8 mm in the sham group.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The between-group mean difference was \u22125.0 mm, with a 95% confidence interval from \u221214.3 to 4.3 mm and a P value of .59.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That means the trial did not demonstrate a statistically significant additional pain benefit from HILT when added to the stretching program.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Inside the HILT group itself, pain improved substantially.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The mean change in VAS was \u221235.3 mm.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But the sham group also improved by \u221230.4 mm.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is exactly why sham-controlled trials matter.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Without the sham group, a manufacturer could easily point to the 35.3 mm improvement and say that the laser caused the improvement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The study design shows that this conclusion would be too simple.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why Both Groups Improved<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Both groups performed the same plantar fascia and Achilles tendon stretching program.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The stretching was standardized.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients were instructed to pull the forefoot toward the body while maintaining dorsiflexion, holding the stretch for 30 seconds and completing 10 repetitions once per day.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Exercise adherence was monitored through patient logs.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That gives us a useful clinical lesson.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The patient&#8217;s improvement may come from the rehabilitation program itself.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The
