La plaie s'\u00e9coulait constamment d'un liquide s\u00e9reux et ne pr\u00e9sentait aucun signe d'\u00e9pith\u00e9lialisation depuis plus d'un mois. Elle suivait une radioth\u00e9rapie adjuvante, qui entrave encore la cicatrisation de la plaie. Les soins standards de la plaie impliquant des pansements avanc\u00e9s et des agents topiques n'ont pas permis de faire progresser la cicatrisation. Elle pr\u00e9sentait un risque \u00e9lev\u00e9 d'infection et de nouvelle intervention chirurgicale.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Constatations objectives :<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>\u00c9valuation des plaies :<\/strong>\n<ul class=\"wp-block-list\">\n<li><strong>Localisation :<\/strong>\u00a0Axe gauche.<\/li>\n\n\n\n<li><strong>Taille :<\/strong>\u00a04,0 cm x 2,5 cm.<\/li>\n\n\n\n<li><strong>Profondeur :<\/strong>\u00a00,4 cm.<\/li>\n\n\n\n<li><strong>Lit de la plaie :<\/strong>\u00a080% mucus jaune p\u00e2le, non viable ; 20% tissu de granulation rouge. Aucun signe de migration \u00e9pith\u00e9liale.<\/li>\n\n\n\n<li><strong>Exsudat :<\/strong>\u00a0Drainage s\u00e9reux mod\u00e9r\u00e9.<\/li>\n\n\n\n<li><strong>Peau p\u00e9ri-l\u00e9sionnelle :<\/strong>\u00a0\u00c9ryth\u00e9mateuse (rouge), mac\u00e9r\u00e9e et sensible.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>La douleur :<\/strong>\u00a0Note de 6\/10 sur le site de la plaie, exacerb\u00e9e par les mouvements du bras.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Plan de traitement :<\/strong><br>Un protocole complet de traitement des plaies a \u00e9t\u00e9 mis en place, avec&nbsp;<strong><a href=\"https:\/\/fotonmedix.com\/fr\/categorie\/produit\/\"   title=\"Produits\"  data-wpil-monitor-id=\"2742\">laser de classe 4<\/a> th\u00e9rapie<\/strong>&nbsp;comme principale modalit\u00e9 de biostimulation.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Dispositif :<\/strong>\u00a0A\u00a0<strong><a href=\"https:\/\/fotonmedix.com\/fr\/categorie\/produit\/\"   title=\"Produits\"  data-wpil-monitor-id=\"2741\">classe 4 th\u00e9rapie au laser<\/a><\/strong>\u00a0avec deux longueurs d'onde (905nm puls\u00e9, 810nm continu).<\/li>\n\n\n\n<li><strong>Fr\u00e9quence :<\/strong>\u00a03 fois par semaine.<\/li>\n\n\n\n<li><strong>Protocole :<\/strong>\n<ul class=\"wp-block-list\">\n<li>La plaie a d'abord \u00e9t\u00e9 nettoy\u00e9e et d\u00e9barrass\u00e9e des mucosit\u00e9s qui s'\u00e9taient d\u00e9tach\u00e9es.<\/li>\n\n\n\n<li>Le laser a \u00e9t\u00e9 appliqu\u00e9 de deux mani\u00e8res : 1) directement sur le lit de la plaie selon un sch\u00e9ma en grille, en tenant la pi\u00e8ce \u00e0 main 1 \u00e0 2 cm au-dessus du tissu. 2) Sur le p\u00e9rim\u00e8tre de la plaie pour stimuler la migration \u00e9pith\u00e9liale et le flux lymphatique.<\/li>\n\n\n\n<li>Une dose plus faible (4 J\/cm\u00b2) a \u00e9t\u00e9 utilis\u00e9e au d\u00e9part pour \u00e9viter une surstimulation, puis augment\u00e9e progressivement jusqu'\u00e0 6 J\/cm\u00b2 au fur et \u00e0 mesure que le tissu r\u00e9pondait.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Soins compl\u00e9mentaires :<\/strong>\u00a0Utilisation continue d'un pansement en hydrofibre pour g\u00e9rer l'exsudat et prot\u00e9ger la peau p\u00e9ril\u00e9sionnelle.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>R\u00e9sultats et cons\u00e9quences :<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Apr\u00e8s 3 traitements (1 semaine) :<\/strong>\u00a0Le lit de la plaie a montr\u00e9 une am\u00e9lioration significative, avec une r\u00e9duction du bourrelet \u00e0 30% et une augmentation du tissu de granulation rouge \u00e0 70%. Le drainage a \u00e9t\u00e9 r\u00e9duit.<\/li>\n\n\n\n<li><strong>Apr\u00e8s 6 traitements (2 semaines) :<\/strong>\u00a0La plaie mesurait 3,0 cm x 1,5 cm. La mucosit\u00e9 a \u00e9t\u00e9 compl\u00e8tement \u00e9limin\u00e9e. Des \u00eelots \u00e9pith\u00e9liaux \u00e9taient visibles et les bords de la plaie se contractaient nettement (contraction de la plaie).<\/li>\n\n\n\n<li><strong>Apr\u00e8s 9 traitements (3 semaines) :<\/strong>\u00a0La plaie mesurait 1,0 cm x 0,5 cm. La douleur est \u00e9valu\u00e9e \u00e0 1\/10.<\/li>\n\n\n\n<li><strong>Apr\u00e8s 12 traitements (4 semaines) :<\/strong>\u00a0La plaie \u00e9tait compl\u00e8tement ferm\u00e9e et \u00e9pith\u00e9lialis\u00e9e.<\/li>\n\n\n\n<li><strong>Suivi :<\/strong>\u00a0Les tissus cicatris\u00e9s sont rest\u00e9s forts et r\u00e9sistants pendant toute la dur\u00e9e des radioth\u00e9rapies.
