Pendant des d\u00e9cennies, la ligature et le stripping ont \u00e9t\u00e9 des moyens brutaux pour traiter l'insuffisance veineuse chronique (IVC). Aujourd'hui, le paradigme s'est enti\u00e8rement d\u00e9plac\u00e9 vers l'ablation thermique. En tant qu'ing\u00e9nieurs cliniques et <a href=\"https:\/\/fotonmedix.com\/fr\/categorie\/produit\/\"   title=\"Produits\"  data-wpil-monitor-id=\"2326\">laser m\u00e9dical<\/a> En tant que sp\u00e9cialistes, nous reconnaissons que l\u2019efficacit\u00e9 de l\u2019EVLT ne se r\u00e9sume plus \u00e0 savoir \u201c si \u201d cette technique fonctionne, mais \u201c comment \u201d l\u2019optimiser. Le d\u00e9bat est pass\u00e9 de la simple occlusion aux subtilit\u00e9s physiques des coefficients d\u2019absorption de l\u2019eau, et plus particuli\u00e8rement \u00e0 la pr\u00e9dominance du <strong>Traitement des veines au laser 1470nm<\/strong> combin\u00e9e \u00e0 la technologie d'\u00e9mission radiale. Cet article constitue un dossier technique pour les chirurgiens vasculaires et les directeurs de clinique qui souhaitent am\u00e9liorer leurs protocoles de phl\u00e9bologie.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">La physique de l'occlusion : Pourquoi la longueur d'onde d\u00e9termine la r\u00e9cup\u00e9ration<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">L'objectif fondamental du LEV est de d\u00e9livrer une \u00e9nergie thermique suffisante \u00e0 la paroi de la veine pour provoquer une contraction irr\u00e9versible du collag\u00e8ne et une destruction de l'endoth\u00e9lium, conduisant \u00e0 une fibrose et \u00e0 une \u00e9ventuelle r\u00e9sorption de la veine. Ces longueurs d'onde ciblaient les <strong>h\u00e9moglobine<\/strong>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>M\u00e9canisme :<\/strong> L'\u00e9nergie du laser a fait bouillir le sang, cr\u00e9ant des bulles de vapeur qui ont ensuite \u00e9bouillant\u00e9 la paroi de la veine.<\/li>\n\n\n\n<li><strong>Inconv\u00e9nient :<\/strong> Cette approche dite du \u201c sang bouillant \u201d entra\u00eenait fr\u00e9quemment une perforation veineuse, un h\u00e9matome important (ecchymose) d\u00fb \u00e0 une extravasation et des douleurs postop\u00e9ratoires.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Les protocoles modernes d\u00e9fendent la <strong>Longueur d'onde 1470nm<\/strong>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>M\u00e9canisme :<\/strong> Le coefficient d'absorption de l'eau \u00e0 1470 nm est environ 40 fois plus \u00e9lev\u00e9 qu'\u00e0 980 nm. La paroi veineuse \u00e9tant fortement hydrat\u00e9e (eau interstitielle), l'\u00e9nergie de 1470 nm est absorb\u00e9e directement par la paroi du vaisseau plut\u00f4t que par le sang.<\/li>\n\n\n\n<li><strong>R\u00e9sultat :<\/strong> Cela permet des r\u00e9glages de puissance plus faibles, une n\u00e9crose coagulative pr\u00e9cise de l'endoth\u00e9lium et une r\u00e9duction drastique des dommages thermiques collat\u00e9raux aux nerfs et \u00e0 la peau environnants.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Le syst\u00e8me de distribution : Fibre nue ou fibre radiale<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Le dispositif laser, c'est le moteur, mais la fibre optique, ce sont les pneus : c'est elle qui d\u00e9termine comment la puissance est transmise \u00e0 la route.<\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li><strong>Fibres nues (premi\u00e8re g\u00e9n\u00e9ration) :<\/strong> Celles-ci \u00e9mettent un faisceau dirig\u00e9 vers l'avant. Ce \u201c faisceau lumineux \u201d concentr\u00e9 d'\u00e9nergie provoquait souvent une carbonisation (br\u00fblure) \u00e0 l'extr\u00e9mit\u00e9 et des points chauds localis\u00e9s, augmentant ainsi le risque de perforation de la veine.<\/li>\n\n\n\n<li><strong>Fibres radiales (la norme actuelle) :<\/strong> Ils utilisent une pointe prismatique pour \u00e9mettre de l'\u00e9nergie dans un anneau de 360 degr\u00e9s (ou double anneau). Cela garantit une irradiation uniforme de toute la circonf\u00e9rence de la veine. L'\u00e9chographie a confirm\u00e9 le spasme imm\u00e9diat et la fermeture de la veine derri\u00e8re la pointe. La lourdeur des jambes a disparu.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Conclusion clinique :<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L'utilisation d'une longueur d'onde de 1 470 nm a permis d'\u00e9viter la formation de \u201c cordons \u201d et les ecchymoses g\u00e9n\u00e9ralement associ\u00e9es aux anciennes techniques d'ablation \u00e0 haute temp\u00e9rature. Le patient n'a connu aucun temps d'indisponibilit\u00e9, ce qui met en \u00e9vidence les avantages de l'ablation endoveineuse au laser par rapport au stripping chirurgical.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Analyse \u00e9conomique : Co\u00fbt de la chirurgie de la varice et retour sur investissement de la clinique<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Pour un centre vasculaire, la transition vers le LEV en cabinet est l'un des mouvements strat\u00e9giques les plus rentables qui soient.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">\u00c9conomie compar\u00e9e : EVLT vs. Stripping<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Le d\u00e9pouillement :<\/strong> N\u00e9cessite une salle d'op\u00e9ration, une anesth\u00e9sie g\u00e9n\u00e9rale ou rachidienne, une \u00e9quipe chirurgicale et une p\u00e9riode de convalescence importante (co\u00fbts des lits d'h\u00f4pitaux).<\/li>\n\n\n\n<li><strong>EVLT :<\/strong> R\u00e9alis\u00e9e dans une salle d'op\u00e9ration standard, elle ne n\u00e9cessite qu'une anesth\u00e9sie locale par tumescence, un chirurgien et une infirmi\u00e8re\/sonographe.
