The management of chronic lower back pain, specifically when complicated by radiculopathy or sciatica, has undergone a radical transformation over the last two decades. For a significant portion of my twenty-year career in clinical laser medicine, the primary challenge was not the lack of biological intent, but the limitation of hardware. Light is an electromagnetic wave that, upon entering biological tissue, is subject to absorption by chromophores and scattering by cellular membranes and extracellular fibers. According to the inverse square law and the principles of light transport in turbid media, the number of photons reaching a specific depth decreases exponentially. This lack of oxygen leads to the binding of Nitric Oxide (NO) to the CCO enzyme, which effectively “clogs” the electron transport chain.
